CHEST 2009 Registration Information

Two Ways to Register

 Only online registration will be available October 10 through October 29.
 On-site registration will open October 30.

 

 On-site registration will be available at the San Diego Convention Center.

Registration Information

  • Registrants with an ACCP membership application on file will receive the membership discount. Learn About ACCP Membership

  • Each registrant must complete a registration form. A photocopy of the form is acceptable.

  • Registrations must include full payment or credit card information. Payment must be made in US funds and drawn on a US bank. Registrants outside the United States using a credit card will be charged at the exchange rate effective the date the charge is applied.

  • US and Canadian faculty and attendees who register and pay in full by September 28 will receive a yellow packet by priority mail the week of October 12. The packet will include your badge, guest badge(s), tickets, ribbons, and claim ticket for the final program and registration bag. Bring the entire packet to the ACCP express check-in counters at the San Diego Convention Center.

  • US and Canadian attendees who register and pay after September 28 can pick up registration packets at the ACCP registration counter at the San Diego Convention Center.

  • All international attendees and faculty can pick up registration packets at the ACCP registration counter at the San Diego Convention Center.

 

International Registration

  • After registering for CHEST 2009, international registrants will receive a letter of invitation to facilitate the visa approval process. CHEST 2009 is posted on the US Department of State’s Intranet listing for conferences in the United States. For travel information, contact US Immigration Support or the US Customs and Border Protection.

  • See group rates and polices for a group of five or more attendees.

  • All international attendees and faculty can pick up registration packets at the ACCP registration counter at the San Diego Convention Center.

 

ACCP Simulation Center Registration

Separate online registration and fees are required for participation in scheduled sessions in the ACCP Simulation Center. If you register for CHEST 2009 by mail, fax, or phone, you must register online for a scheduled session. Space is limited, so register early. No refunds or exchanges will be permitted.

Postgraduate Course Registration

Register for postgraduate courses using the CHEST 2009 registration form. Indicate the course(s) you want to attend and note the additional fees. Space is limited, so register early.

Guest Registration

Guests 18 years or older who are not paid Ambassadors Group members by October 1 will be charged $75. Registration for guests younger than 18 years is free. Guests will receive a badge and will be allowed to attend the special events and visit the exhibit hall. Anyone wishing to claim CME credits must register as an attendee. Refunds will not be given for guest registrations.

 

Member Spouse for CME Registration

Spouses of ACCP members may register for CHEST 2009 to earn CME credit. To register as a member spouse, contact ACCP Customer Relations at (800) 343-2227 or (847) 498-1400 or register on-site.

 

Refunds

ACCP Simulation Center registration and the guest fee are nonrefundable. Other refund requests must be received in writing, on or before October 9, for a refund less a $50 processing fee. Refunds will not be issued after October 9.
