Case Report Submission Information and Guidelines
Information
- ACCP affiliate members may submit multiple case reports, but only one will be accepted for presentation. The Affiliate Committee will select case reports based on originality, well-defined pathophysiology, clinical relevance, and differential diagnosis of each case. Affiliate members attending the ACCP Fellows Conferences are especially urged to participate.
- Use the online submission site to submit case reports. For the best results, use Internet Explorer 6.0 or higher. Program enhancements may not be available if a lower version of Netscape or Internet Explorer is used.
- You can enter your case report, save it, and return for edits or updates anytime prior to the deadline. Final submission must be made on or before May 4, 2009, at 12:00 pm (CT).
- After the submission deadline, all completed and submitted case reports will be sent for review. Incomplete case reports will be discarded.
- Electronic confirmation of your Case Report Author Agreement must accompany your submission. This agreement acknowledges that reports relating to human research have been subject to investigational protocol review and approval by a formally constituted institutional review board for human studies.
- Receipt of case reports will be acknowledged electronically; phone acknowledgments will not be made.

Guidelines
Case reports must comply with the guidelines below. The ACCP reserves the right to reject case reports for any reason, including failure to meet the submission requirements.
- The primary or presenting author of accepted case reports must be an affiliate member of the ACCP at the time the case report is submitted. Nonmembers may apply for membership until April 27, 2009, to be considered eligible. Contact Christina Vock for more information.
- Case reports must be submitted and presented in English.
- Material submitted must be original and may not be presented or published elsewhere in abstract form prior to November 5, 2009.
- Submission of a case report constitutes the author’s intent to present at CHEST 2009. Presentations should be well-documented and include radiographs, pathologic specimens, and laboratory values, as appropriate.
- The case report must be titled to define the topic and contain no abbreviations. Standard abbreviations may be used in the text of the case report. On first use, the full term should be spelled out with the abbreviation in parentheses. Author names are not allowed in the title of the case report; author names should be entered in the designated author information area.
- All submissions must be HIPAA-compliant. All individually identifiable health information is to remain private. Review HIPAA guidelines.
- The body of the case report must contain five separate parts: introduction, case presentation, discussion, conclusion, and two references (optional).
- The total word count may not exceed 600 words. Only two references will be permitted. References are included in the word count.
- Commercial logos are prohibited from appearing on case report slide presentations.
- Disclosure information must be provided. If presenters have nothing to disclose, this must be indicated. Failure to disclose grant support or funding will disqualify a case report.
Notification of acceptance or rejection will be made after July 10, 2009. If you have not been notified by July 20, 2009, contact the ACCP. If you do not accept or decline to present at CHEST 2009 by August 3, 2009, your case report will be discarded.
